- Sleep Disorders Clinic is a custodian of and accountable for any personal and personal health information that is under its control and that of its agent. The Privacy Officer at Sleep Disorders Clinic is responsible for implementing and monitoring compliance with the privacy principles that are reflected in our policies and practices.
- We collect, use, and disclose personal and personal health information of clients for the main purpose of providing you diagnostic, health and treatment information and options, except when you are asked to sign an additional consent form.
