If you are the parent or guardian of a child under the age of 16, you will need to bring the
following form with you to the appointment. Patients under age 16, a parent or guardian is
required to fill out the questionnaire prior to your appointment.
Please bring the completed questionnaire to your appointment. In addition, we require you to fill
in a two week sleep diary for your child.
